Your Visit
Schedule an Appointment
To serve you in the best way possible, we work by appointments. Please let us know of any issues that may necessitate how soon you need to be seen.
At your initial visit, you will be asked to present your insurance card, photo ID, any necessary co-payments and to complete new patient paperwork. To save time during your first appointment, you may download and complete our new patient paperwork ahead of time and bring it with you to your appointment.
To Cancel or Reschedule
If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment at least 24 hours prior to your scheduled appointment time.
Policies and Instructions
Ancillary Tests
For our patients' convenience, we offer Mammography, Bone Density and Ultrasound testing on-site. We ask that you make necessary childcare arrangements prior to your testing. For safety and liability reasons, we do not permit children in the testing rooms. Children may be left in the waiting room, provided an adult accompanies them. Please note that technicians are not authorized to discuss test results.
Laboratory Service
Lab specimens collected in our office are sent to an outside laboratory for testing with the exception of pregnancy tests, routine urinalysis and hemocults.
Prescription Refills
For routine prescription refills, please leave a message on our prescription refill line. If your call is received prior to 4:00 p.m., your prescription will be called into the pharmacy you indicate within 48 hours. Our office policy is not to refill birth control prescriptions or non-emergent medication during the hours when our office is closed. We ask our patients be responsible for monitoring prescription needs. Please remember that the physician is on-call for emergency purposes only.
Non-patients
We cannot answer clinical questions or give medical advice to any patient who has not been seen at this office during the previous three years. Patients who have not been seen for the past 12 months or longer are kindly asked to schedule an annual exam. We reserve the right to provide care for our patients only.
