Your Visit
Schedule an Appointment
To serve you in the best way possible, we work by scheduled appointments. We suggest you call prior to the time you wish to be seen for routine and annual appointments. Please let us know of any issues that indicate that you need to be seen sooner as we have same day appointments for emergencies.
To Cancel or Reschedule
If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment at least 24 hours prior to your scheduled appointment time in order to avoid a missed appointment charge. We greatly appreciate this consideration.
Insurance
Insurance Plan Participation
At Physicians for Women we participate with most insurance plans but suggest that you check with your insurance company to verify our participation in your specific plan.
At your initial visit, you will be asked to present your insurance card, photo ID, and to complete new patient paperwork. After scheduling your first appointment you will be mailed the necessary new patient forms or you may download the necessary forms here.
