Your Visit
Schedule an Appointment
To serve you in the best way possible, we work by appointments. Please let us know of any issues that may necessitate how soon you need to be seen. If we are unable to answer, please leave a message and we will return your call as soon as possible.
At your initial visit, you will be asked to present your photo ID, insurance card, and any necessary co-payments as well as complete new patient paperwork. To save time during your first appointment, you may download and complete our new patient paperwork prior to your visit and bring it with you to your appointment.
To Cancel or Reschedule
If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment within 24 hours of your scheduled appointment time.
In Case of Emergency
For matters of an urgent nature please call 860-210-0082 and follow the instructions to page our on-call physician. For true emergencies please call 911.
New Patients
We ask all new patients to please request a copy of her medical records from your previous physician prior to the first appointment with Women's Healthcare of New Milford. A Medical Records Release authorizes your previous physician to send this information to us. Please be sure to bring any necessary forms as well as your insurance card, a photo ID and applicable co-payment to your first appointment.
Insurance
Insurance Plan Participation
At Women's Healthcare of New Milford we participate in many managed care plans and see patients with government as well as traditional insurance. Some insurance plans may require that you obtain a referral from your regular doctor (PCP) before you visit us if we are not your designated gynecologist. To learn if a particular physician participates with your plan, please contact our office or your insurance company.

Billing and Payments
You are responsible for learning details of your coverage from your insurance plan, including all deductibles, co-payments and/or any referrals that may be required by your insurance company. Your co-payment is expected at the time of service, and payment in full is expected at each visit for services not covered by your insurance. Additionally, if you do not have an insurance card available, and/or our physicians do not participate with your health plan, full payment also is expected at the time of service.
For your convenience, we accept cash, check, Mastercard, VISA, Discover and AMEX. All returned checks will be assessed a $25.00 service charge. If you are an existing patient with a balance due, you can also pay your bill online.

For your convenience, Women's Healthcare of New Milford provides the following forms that you may download and complete before visiting us. Otherwise, the applicable forms will need to be filled out upon arrival at our office. Should you opt to complete them at our office, please arrive at least 15 minutes prior to your appointment. At regular intervals, you will be asked to update this information.
